  Opening A New Wholesale Account   

Customer Registration Form

If you wish to open a wholesale account with us, please complete the following form and provide appropriate documentation so we can get your account set up.  

In order to receive wholesale pricing, product information, and ordering, we require:

Mandatory Requirement:  Provide proof of one of these 2 options :

Option 1: A picture of your valid Professional license certificate/card or

Option 2: A picture of the Beauty School license,

Additional supporting information is optional:

• A picture of the Resale Tax Certificate, 
• Proof of your official business website: i.e. your business website address

Professional license options are (aesthetician, cosmetologist, nail technician, massage therapist, beauty school, R.N., M.D., LPN).  We do not accept account applications from students.

Please email a copy or picture of the professional license to:  Admin@primeralabs.com

Resale Tax Certificate: Please email a copy or picture of a valid resale tax certificate. We do not accept a Federal EIN(Employer Identification Number).

Please email a picture of the Sales Tax Certificate to:  Admin@primeralabs.com

Business Website: We do not accept wholesale applications for those who have not yet officially fully developed their website. 

Once we receive, review and approve your appropriate document, we will email you account access in 5 business days( excluding weekends and holidays).
